VTA Sick Leave donation requests
Run out of sick days and need to be out of the classroom for an extended time?
Interested in requesting sick days from your colleagues to cover the time you are out?
This is what you need to do:
1. Get a copy of your doctor's note that lists the time they have signed you off from work for verification.
2. Fill out the form below to notify the VTA Office of your request.
3. Scan or fax your doctor note to the VTA Office.
The VTA Office will then look at your request to make sure it qualifies for donations. The VTA President will sign off the request and send it to HR for approval. Once the approval form is sent back to the VTA Office, there are a few ways we can distribute the donation forms:
1. Send to your school site
2. Send to specific people of your request
3. Send to other school sites you may know people or be affiliated with.
After distributed, wait a week before calling Payroll to inquire if anyone has donated days to your account yet. VTA does not get notification of donated days.
How sick leave is used:
Once you exhaust all of your sick leave, the District Office will place you on Differential Pay. You can stay on differential pay for 100 days. Differential pay is your normal day's pay, minus the sub cost. If you receive sick leave donations, they will give you a full day pay instead of Differential's pay.
If you receive more donated days than the amount you actually use, the days will be returned back to the people whose days you did not use.
